Health Services at Columbia Reimbursement Procedure

As a Barnard Student, there are certain times during the year when you have access to urgent care services at Health Services at Columbia (3rd floor of John Jay Hall), such as winter, spring, and summer breaks. You will be charged $60.00 for every visit to Health Services at Columbia, which must be paid at the time of your visit. However, the $60.00 fee can be reimbursed by Student Insurance. Please note that if you visit Health Services at Columbia during the summer when Barnard Student Health Services is closed, you MUST be covered by Student Insurance to be reimbursed. Please call BC Student Health Services for more information about summer insurance coverage. You may also claim the $60 fee to your family insurance.

Below is the step-by-step reimbursement procedure:

While at Health Services at Columbia, be sure to ask your provider for a receipt that contains all the codes for your visit (CPT & ICD-9), the Health Service Tax ID#, the provider's name and title and the date of your visit.
If you will be claiming the $60 fee to Student Insurance, visit the BC Student Health Service as soon as possible—during winter, spring, and summer breaks BCSHS is open for administrative care—to pick up an Insurance Claim Form. Simply ask the receptionist for a form. Feel free to ask for more than one form for future use.

After you have completed the Insurance Claim form, mail it along with your receipt from Columbia and your written diagnosis to:

If you are claiming the $60 fee to your individual family insurance, you will have to submit a claim form directly to your insurance company.

Please Note: The Barnard College Student Health Service cannot reimburse you directly. Reimbursement is available through Student Insurance or your individual family insurance only.
