FILING INSURANCE CLAIMS

- 1. Be sure you have a
Super
Bill from the clinician or institution (e.g., emergency
room, x-ray facility, laboratory) where you received care. A Super
Bill gives the date you were seen, a specific diagnosis with a code,
and the name of the clinician who treated you.

- 2. Find out if your
parent's medical insurance or Medicaid covers you. If yes, go to Step 3; if no, go to
Step 6. For reimbursements for a visit to Health Services at
Columbia, go to Step 6 (you do not need to file with your
parental insurance).
3. Submit one copy of the Super Bill with the claim
form from your parent's insurance company or Medicaid to the indicated address, filing out the
requested information. If they ask for date of service, diagnosis, etc.,
you can put "Please see attached bill." Be sure to
sign so that your clinician can be paid directly!
- 4. You will receive
notification from your parent's insurance company or Medicaid telling you how much of the
bill has been paid. If your bill has not been totally paid, proceed
to Step 5.

- 5. Submit one copy of
the Super Bill and a copy of the explanation of payment from your
parent's insurance company or Medicaid, and a Student Insurance
claim form (available in
Health Services) to the address listed on the insurance
form.

- 6. If Student Insurance is your
only coverage, submit one copy of the Super Bill or your receipt
from Health Services at Columbia and the
insurance form (available in Health Services) to
the address listed on the insurance form. Fill
in the requested information. For diagnosis, date of service, etc.,
you can put "Please see attached bill." If you have only a receipt,
you must fill out this information. Except for
visits to Health Services at Columbia, you must
complete the section about other/parental insurance or your claim will not be processed.

- PLEASE KEEP COPIES OF ALL
DOCUMENTATION.

- You may contact Barnard College
Student Health Services at extension 4-2091 if there are
problems getting your claim paid.

- REMEMBER, visits to the emergency room
or to consultants must be authorized
by a clinician in Health Services or by the clinician-on-call for
Student Insurance to be used. The Clinician-On-Call
can be reached 24 hours/day whenever
Health Services is closed at 866-966-7788
throughout the academic year, including Thanksgiving, Winter
and Spring Breaks.
