Referral to a Specialist:
What You Need to Know
You are being referred to a specialist.
A combination of parental and Student Insurance will pay
towards your visit if you follow the steps below:
Bring the following items with you to your appointment:
1) A completed three piece (triplicate) "Referral to
Consultant" form or a referral to consultant form from
your Barnard clinician.
2) A claim form for the
UnitedHealthCare StudentResources (the
Barnard College Student Injury and Sickness Insurance Plan.) You
may pick one up at the Front Desk or ask your
clinician for one.
3) If you are covered by
parental or family insurance, an insurance member ID card and a
claim form from your parental or family
insurance policy, signed by the policyholder (i.e., your parent)
if required by that insurance company.
4) Approval from the
Director of Health Services if you are
being referred for surgery.
Remember…
If your parental or family insurance requires that you see only physicians
in their network, you should schedule your appointment with
them, or obtain the appropriate authorizations from them well in
advance to see a physician who is "out of network."
If you are not sure if your parental or family insurance will cover you
visit, you will need to find out prior to making your
appointment. You still must submit your bills to your parental
insurance before sending them to
UnitedHealthcare StudentResources . You will receive an
Explanation of Benefits (EOB) from them stating how much, if
anything, was paid.
Balances left over after parental insurance has paid:
some portion will be evaluated by Student Insurance if a Barnard
clinician initially approved the referral and you properly file a
claim with UnitedHealthcare StudentResources (usually within 90 days of
treatment).
