a) The contact should be someone who can answer any questions regarding this request.

Second, please complete a TR-MAC Request Form as follows. If you are emailing your request to purchasing, an authorized member of your department need only send the following information.

1. Request Date
2. Requisitioner's contact information (name, extension and email)
3. Requisitioning Department
4. Account number work should be charged to
5. BC number (a 5 digit number starting with "95" found on your phone bills)
6. Description of work to be done. Be as detailed as possible and include all available information such as floor plans. Please do not put requests for several different jobs on the same requisitions as this may cause a delay.

**Please note that for requests to reset voice mail or change the name on an extension, the only information necessary is the extension and/or name change, as these changes as done free of charge. We recommend this information be submitted via email

Return a completed and signed TR-MAC form to the Purchasing Office or email request to purchase@barnard.edu . Please do not send your requests to specific individuals in the Purchasing Office as that also may delay service.
