Forms

Many Registrar's Office forms are multi-part and must be picked up at the Registrar's Office.

The forms listed below are available online, if you have Adobe Acrobat Reader.  You may fill them out online, print them out, get any signatures that may be required, then submit to the Office of the Registrar, 107 Milbank.

These forms cannot be submitted online.  Please read all forms carefully.

Application for an Incomplete -- This is a two-sided form and must be completed in triplicate; make sure to print out three copies of both sides.  Instructor's signature required.

Course approval application -- Upon returning to Barnard after studying elsewhere, you may use this form to request credit for courses taken.

Deferred Exam Form, Spring 2008 -- Deferred final examinations for Barnard students are open only to those students who were absent from the regular examinations for reason of illness or emergency. All illnesses and emergencies must be reported to the Office of the Dean of Studies in accordance with Dean Blank's memorandum. To be eligible to take a deferred exam, you must have your name appear on the Dean of Studies approval list. The form must be filed with the Registrar's office no later than August 4, 2008.

L Override

Request for approval of credit for independent study -- This form cannot be filled out online.  It may be printed out, filled out, then turned in to the Registrar's Office, 107 Milbank, with signature of faculty sponsor and Barnard department chair.

Request for evaluation of college work completed prior to admission to Barnard -- First-year students with a record of prior coursework taken as non-matriculants at an accredited college in the United States may request up to 15 points of transfer credit. Such work will be evaluated after the student has completed 12 points at Barnard.  The courses must be intended primarily for college students and taught at the college by members of its faculty. With the exception of Advanced Placement courses overseen by the College Board, courses taught in a high school, either by specially trained high school teachers or by college instructors, will not be credited toward the Barnard degree.    No adviser's signature required.

Request for exception to the elementary language rule -- Although credit for the first semester of an elementary language is not normally granted unless a more advanced course is completed, a student is granted one exception maximum to this rule on written request to the Registrar.   No adviser's signature required.

Request for a point for dance technique class -- If you have completed your physical education requirement, you may request a point for a dance technique class.  No adviser's signature required.  Please note that we cannot adjust point value until your adviser has approved your program.

Summer school approval form --  To be credited toward the degree, courses must be given at accredited institutions.  Courses should meet for at least 5 weeks and must meet for at least 35 hours.  If your proposed course does not meet these criteria, you must file the Summer Petition.  No undergraduate lecture course that meets for under 35 hours will be approved.

Variable-point courses -- When you add a variable-point course to your program, the credit defaults to the lowest value.  If you are taking this course for a higher value, you must submit this form.   No adviser's signature required.  Please note that we cannot adjust point value until your adviser has approved your program.
